Transition and Partnership Officer

We are currently seeking a suitably qualified individual to join our team as a Transition and Partnership Officer within the Additional Learning Needs Support Service. The successful applicant will join a busy, friendly and innovative team of ALNSS officers, who are committed to assisting the LA in discharging its statutory duties in accordance with legislation.

As a Transition and Partnership Officer, you will have a sound knowledge and experience of Person-Centred Practice (PCP) and the requirements of the Additional Learning Needs Education and Tribunal (Wales) Act, 2018, along with a commitment to securing high quality service delivery for all children and young people with Additional Learning Needs (ALN) and their families.

Your daily routine will vary and involve meetings with schools and professionals, support for families, transition support for children and young people across NPT and office working.

Do you have:
  • A minimum of 3 years' experience of working with / supporting children and/or young people with Additional Learning Needs (ALN) and their families.
  • Experience of working across the age spectrum including pre-school through to post 16.
  • A thorough understanding of Person-Centred Practices including knowledge of person-centred transition planning and review processes.
  • Commitment to working in a busy, challenging and changing environment.
  • Excellent organisational, time management and planning skills.

School based teaching and TA staff, those with experience of working with children and young people with ALN, Youth Workers with ALN experience, applicants who have experience working with families and are familiar with multi-agency working.
